Required Documents

For Modified, Street Rod & Custom Automobile Insurance™

Please refer to the following list of required documents.

1. A completed Application form (Custom Wheel Application Form (pdf) »)
2. An Appraisal (including color photos).
3. A completed Privacy Consent Form (click here to download form »)
4. A copy of the Pink Card from your daily driver.
5. A three year Driver Abstract for all operators.
6. Proof of ownership (Bill of Sale or Registration).
7. A cheque for the appropriate amount (payable to Aviva/Elite Insurance).

Upon receipt of the above we will confirm that your coverage is in place.
